BROODMARE DECLARATION FORM FOR STALLION SERVICE BUYERS

AT THE 2009 ALL AMERICAN CUP AUCTION

This form is to be used by the Broodmare Owner who purchased a STALLION SERVICE at
THE ALL  AMERICAN CUP AUCTION in January 2009.  The foal from the DECLARED will be
eligible to compete in THE ALL AMERICAN CUP as a Weanling September 2010 and as a
Three Year Old in 2013.

All applicable fees and payments must be made as specified in the Rules and Regulations.  
Your foal must be kept eligible in the Indiana Futurity as a Weanling, Yearling, and Two
Year Old.

The person making the DECLARATION on this form may, or may not, be the Owner of Record
of the mare being DECLARED.  At the time the prize money is to be distributed, the check
and IRS 1099 will be made out and distributed to the person who has "PAID" the
DECLARATION FEE.

Please complete and return the DECLARATION FORM on the 2nd page.

Attach a copy of the REGISTRATION PAPERS of the Mare that is being DECLARED.

The above-mentioned documents must be post marked and received by December 1, 2009.
